• Letter from your employer explaining nature and duration of your business trip to Ukraine. (1)
• Invitation letter from the host company in Ukraine (1)(2)
• Passport that meets requirements set by the Embassy of Ukraine in London [Explained]
• 3 Colour Passport Photographs
• Complete and sign the Ukrainian Visa application form [Download]
• Complete and sign the "The Schengen Office Case Sheet" and "The Schengen Office Terms and Conditions" [Download]
• The Schengen Office Handling Fee: 55 GBP
• Embassy Fee [See below]

Note(1): Not required by nationals of the EU countries, Canada, Japan, Slovak Republic, Swiss Confederation, Republic of Turkey and the USA.
Note(2): Required only if applying for multiple entries.
