*Note: Authorised, signed, stamped with conformation of hotel booking in Ukraine. Must include full name of applicant and duration of stay. Round trip ticket by any Ukrainian airline company can be used instead by nationals of the EU countries, Canada, Japan, Slovak Republic, Swiss Confederation, Republic of Turkey and the USA.
